Improving the health of school aged children
2011
In January 2011, Rocky Mountain Health Plans Foundation will award multiple grants totaling $14,000 to schools who want to implement an approach or purchase school equipment that positively influences the physical health of their student population. Examples of physical health grants given by the Foundation are:
• Step-counters and walking programs
• Climbing wall implemented in gymnasium
• Swim lessons for high school students to learn to swim
• Phil Lawler “Spark” Program
Grant requests from $500 - $3,500 can be submitted to the Foundation by end of day on Friday, December 17, 2010. Grant application is attached.
These grant dollars are raised by employees of the management company of Rocky Mountain Health Plans with their intent to make a difference in the lives of Coloradans. They are pleased to directly serve the youth within our schools for this grant release.

• School contact person, email address and telephone number
• Describe what you want to do with the funding and how it will help improve the health of your students. (500 words or less)
• Timeframe (if appropriate)
• Total dollar amount being requested and details of expenditures
• How will you determine and report the success or outcomes from this grant funding?
• Percent of school population eligible for free and reduced lunches
• Letter of support from School Principal
