Below is a nomination form with a link to the relevant criteria information for the 2009 COAHPERD Recognition Awards. Note that there are a wide variety of award categories. It is the intent of COAHPERD to recognize the excellent instruction of those in our field through these awards. To this end, you are not merely invited but, rather, urged to use the form to nominate those who personify such excellence.

To submit a nomination
- Download and complete the COAHPERD award nomination form
- Email Clayton Ellis at ceellis@aps.k12.co.us
- The completed COAHPERD award nomination form
- Two letters of recommendation for the nominee which speak directly to the guidelines.
- A current resume for the nominee
Deadline to submit is April 24th, 2009
Award winners will be notified in June 2009. Both the winner and his/her nominator(s) will be recognized at the awards banquet during the State Convention.
