Below is a nomination form with a link to the relevant criteria information for the 2010 COAHPERD Recognition Awards. Note that there are a wide variety of award categories. It is the intent of COAHPERD to recognize the excellent instruction of those in our field through these awards. To this end, you are not merely invited but, rather, urged to use the form to nominate those who personify such excellence.

1. Download and complete the COAHPERD Award Nomination Form
2. Email Patricia Morrison Hughes with any questions at pat@coahperd.org
3. Deadline to submit is May 5th, 2010

Award winners will be notified in June 2010. Both the winner and his/her nominator(s) will be recognized at the awards banquet during the State Convention.
